The University is no longer printing packs of Grade Change forms for departmental purchase.  All Grade Change forms must be printed off of the Registrar's site.  They are available from the Faculty & Staff link on the Registrar's page.

You will find the forms by following the "Frequently Used Forms" link (3/4 of the way down the page).  When you follow this link, you will be required to enter your name.# and password to access the Grade Change form.  The Grade Change form is the first one on the page.  "Grades".  You will need to print the form, fill it out and turn it in to USS.  We copy them so our department has a record of the grade change, and then batch them over to the College Office.  These forms can be turned in to any of the staff in USS.
